You’ve never seen a cyst like this. In a new Instagram video, Dr. Pimple Popper—aka dermatologist and TLC host Dr. Sandra Lee, MD—removes a giant pilar cyst from a patient’s head in one piece. It seems pretty unremarkable at first, but what’s inside has Dr. Pimple Popper fans reacting in shock and horror.

In the clip, Dr. Lee makes an incision into the patient’s scalp and works to free the bulbous pilar cyst in one piece. Removing the cyst without puncturing it pretty much guarantees the growth will not return. With careful snips and some expert mauvering, the famed derm is able to pluck the mass out whole, an impressive feat.

After stitching the patient up, Dr. Lee decides to cut open the pilar cyst in front of the patient, revealing its contents. And what comes out looks like liquid chocolate.

“I’m glad I didn’t open that up,” Dr. Lee tells the patient as she cuts the cyst. “That would have been a mess. Look!”

In the caption of the clip, Dr. Lee writes the growth proof that not all pilar cysts are the same.

One fan commented, “What the hell came out of that cyst?!” Another simply wrote, “Now I want chocolate.”

Pilar cysts are growths on the scalp that contain excess keratin, which is the protein that makes up your hair and nails. They are benign, and usually are not painful. Cysts often have white contents, but can be darker in color depending on the melanin content in a person’s skin.
